in communities and institutions.
Coalition
for Healthier Cities and Communities
The Coalition is a partnership of entities from the public, private
and nonprofit sectors whose mission is to "stimulate and encourage collaborative
action and efficient use of resources from multiple sectors and community systems."
The site has information on their agenda, a states network, community forums,
a library resource and news.
Community
Health Status Indicators Web Site
This site is the result of a HRSA funded collaboration among numerous public
health organizations. The purpose of this site is to provide health information
for each county. While the information is compiled from existing reports, the
site provides the information together and compares it to peer counties. This
is a great site for communities to assess their health status and how they compare
to other counties.
Community
Tool Box
The Community Tool Box "is intended for everyone addressing issues
in local communities. The core of the Tool Box is the 'how-to tools.' These
how-to sections use simple, friendly language to explain how to do the different
tasks necessary for community health and development. For instance, there are
sections on leadership, strategic planning, community assessment, advocacy,
grant writing and evaluation. Each section includes a description of the task,
advantages of doing it, step-by-step guidelines, examples, checklists of points
to review and training materials." In addition, there are links to other
web sites and listservs relating to the topics in the Tool Box.
Community
Voices
Community Voices is an initiative from W.K. Kellogg Foundation that contains
information on national health policy and efforts of local communities to improve
health care access and quality. There are reports on minority health initiatives,
oral health, youth violence, mental health and issues related to health care
access. The site features monthly articles by prominent scholars and politicians
as well as serves as a gateway to other important health and nonprofit web sites.
Community-Campus
Partnerships for Health
Community-Campus Partnerships for Health (CCPH) is a nonprofit organization
that promotes health through partnerships between communities and higher educational
institutions. They are a growing a network of over 1000 communities and campuses
who collaborate to promote health through service-learning, community-based
research, community service and other partnership strategies. The site provides
links to membership information, resources, programs, and publications.
Environmental
Maps (E-Maps)
This service, from the Department of Housing and Urban Development (HUD) and
the Environmental Protection Agency (EPA), "is a free Internet service
that combines information on HUD’s community development and housing programs
with EPA’s environmental data. HUD E-MAPS provides: location, type and performance
of HUD-funded activities in every neighborhood across the country; and select
EPA information on brownfields, hazardous wastes, air pollution and waste water
discharges."
Grass-roots.org
Site contains stories and real life examples of "innovative grassroots
programs in the United States and the local heroes who've found effective ways
to build their communities, fix what's broken and make them better." It
also contains links to grassroots and community groups as well as national and
world groups.
Guide to Community
Preventive Services
The Community Guide provides a resource to improve commuinity health outcomes for such programs as tobacco control, alcohol, cancer, physical activity, substance abuse, and oral health.
Health
Policy Coach
Health Policy Coach is part of the California Center for Health Improvement
(CCHI). The Coach "is a resource for people seeking to improve community
health through prevention-focused policy change. The site provides peer-reviewed,
searchable health policies and "how-to" information and coaching resources
of individuals and groups interested in effecting policy change in their communities."
Policies include such topics as child care, tobacco, firearms, environmental
toxins, illegal drugs, alcohol, and infectious disease.
John
D. and Catherine T. MacArthur Foundation
The MacArthur Foundation sponsors programs on Human and Community Development.
These programs focus on four themes: Access to Economic Opportunity, Building
Community Capacity, Child and Youth Development, and Mental Health. Their Internet
site also contains links for Human and Community Development web sites.

The W. K. Kellogg Foundation is a nonprofit organization whose mission is
to apply knowledge to solve the problems of people. The site contains links
to their areas of interest, which include: health, philanthropy and volunteerism,
food systems and rural development, youth and education/higher education, leadership,
information systems and technology, social and economic community development,
and capitalizing on diversity.
National
Council for Public-Private Partnerships
This organization works for "more effective delivery of public services
through partnership and that helps to develop creative combination of private
and public capabilities." There is membership, workshops, resource &
meeting information at their web site.
The
Trust for America's Health (TFAH)
TFAH is a national nonprofit organization whose mission is to protect the health
and safety of all communities, especially those most at risk of environmental
and other public health threats. Their goal is to strengthen the nation's public
health system through science-based research, community partnerships, education,
and advocacy. The site contains links to state tracking factsheets, issues &
campaigns, research & resources, and a community health forum.
